Position Overview:

The Social Worker/Access assists clients in applying for public assistance programs through the Department of Children and Families' ACCESS initiative. This position involves counseling patients, maintaining records and ensuring access to essential social services.

Key responsibilities:

  • Assist clients with certification and recertification of programs such as:some text
    • Medicaid
    • Food stamps
    • Temporary financial assistance
    • SafeLink wireless program
    • LIS (Low Income Subsidy - Medicare Extra Help Program)
    • QMB (Qualified Medicare Beneficiary)
    • Medicaid Waiver
  • Supports Medicaid and Medicare enrollment.
  • Maintain detailed records, including weekly productivity reports and patient visit records.
  • Assist patients in accessing services such as housing, food and citizenship applications.
  • Encourages patient participation in beneficial activities.
  • Provide information and support to patients and their families.
  • Follows up with discharged patients to ensure satisfaction and outcomes.
  • Act as an advocate for patients' social needs and communicate effectively with care teams.
  • Perform additional duties as assigned by the Center Administrator.

Qualifications:

  • High School Diploma or GED.
  • Great communication skills, attention to detail and customer service.
  • Knowledge of Medicaid and government programs.
  • Proficiency in Microsoft Office.
  • Preferably bilingual (Spanish/English).
